At Hebrews 4:16 Counseling, there are two forms that are required by the counselee(s) to be completed and signed in order to obtain counseling.

Personal Data Inventory Form

The first is the Personal Data Inventory. This form serves as an aid for the counselor in getting to know you, your history and the issue(s) that bring you to counseling. This form can be complete on-line by clicking the link below.

Release of Confidential Information Form

The second form is titled “Release of Confidential Information” which not only allows confidential release of information but is also serves as a liability release form as well. This document needs to be read and signed by the counselee(s). Click the button below to open, print, read and sign this document. You must bring it to your first counseling session.
